Answer To Question 2: For inpatients; it is the whole of Wiltshire and the surrounding counties (Some Dorset and Hampshire patients). For outpatients; it is primarily mid to north Wiltshire as this area is not current supported by a heart failure service in the community. | |

Answer To Question 4: For outpatients we see less than 5 Face to Face four times per week, and 6 phone calls per day making approximately 42 per week We also do a heart failure ward round with a cardiologist twice per week for inpatients with complex needs; 4-6 per week. | |
