As part of a national initiative all NHS organisations are Smoke Free. Patients, visitors, contractors and staff are not be able to smoke anywhere on an NHS site, including ºÚÁÏÍø District Hospital and any other organisation’s facilities situated on our land. Smoking will also not be allowed in Trust vehicles and in on-site accommodation areas. E-cigarettes and vaping will be allowed. Palliative Care/Hospice
For patients of the Hospice ONLY, arrangements may be made on a case-by-case basis. Please speak to Hospice staff.
You may need to do the following |

1. | Cancel regular deliveries such as milk or newspapers | ✔ |

6. | If you receive a state pension or supplementary benefit you should refer to the instructions on the back of your order book regarding admission to hospital | ✔ |
7. | Tell ward staff in advance of any special needs you may have that could affect your ability to remain independent | ✔ |
8. | Arrange (if possible) for a relative or friend to bring you into hospital so that they can take away any clothes or property that you will not need during your stay | ✔ |
9. | Only bring items that you really need. Please mark personal property with your name so that it can easily be identified. A bedside locker is available for your personal possessions but please remember that space on the ward is limited | ✔ |
10. | Ask your GP to check with your consultant if any of your medication should be stopped before coming into hospital, for example, warfarin, non-steroid-anti-inflammatory-drugs (NSAIDS) such as diclofenac or aspirin, the contraceptive pill or herbal medicine | ✔ |

Your admission letter will tell you where to go when you arrive at the hospital but if you are in any doubt please go to the Reception Desk at the Main Entrance and ask for directions. Volunteer guides may be available to escort you to where you want to go.
When you reach the ward you will be welcomed by a member of the nursing staff or the Ward Clerk who will tell you about the ward and its daily routine.

Preventing Infection

You will need to leave the ward by 10am on the day that you are allowed home. If you are unable to travel this early in the day you will be transferred to the Discharge Lounge. This offers comfortable seating, hot and cold drinks and meals if needed until you are collected. Parking for this area is free via a 'pick up point'.

Before a doctor or other health care professional examines or treats you, they need your consent. Sometimes you can simply tell them whether you agree with their suggestions. However, sometimes a written record of your decision is helpful - for example if your treatment involves sedation or an anaesthetic. You will then be asked to sign a consent form. Before signing, the doctor or nurse will explain what is going to happen. remember to ask questions if you are not sure or do not understand something. If you change your mind, you are entitled to withdraw consent, even after signing the form.
When you arrive you will be asked to complete a hospital patient form. This will be used as part of your medical record. We ask you for information about yourself so that you can receive proper care and treatment. We keep this information, together with details of your care, because it may be needed if we see you again. Details of your past and future treatment will be kept in your medical records. Some sections will be processed on computer, in accordance with the Data Protection Act 1988. Great care is taken to make sure that confidentiality is maintained.
Under Data Protection, you have the right to copies of information held in your medical records. This may include medical photography, x-rays, appointment details or test results. If you would like a copy of the information held in your medical records, please apply in writing to:
